Job summary

Northern Cardinal Rehabilitation and Nursing is seeking an Occupational Therapist (OT) to assess and treat residents, develop individualized plans, and collaborate with the care team to optimize functional outcomes. The role emphasizes patient-centered care and ongoing evaluation in a skilled nursing setting.

The OT will provide direct patient care, educate families, document progress, and participate in interdisciplinary rounds while adhering to privacy and residents' rights policies.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients.
• Document and record patients' condition.
• Educate patients and families in appropriate therapy methods.
• Complete evaluations, weekly progress notes, re-certification and discharge documents.
• Collaborate with all disciplines to provide patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure compliance with confidentiality, HIPAA and residents' rights.
