Job summary

Garden City Health Care Center in Modesto, CA is seeking a dedicated Occupational Therapist (OT) to join our rehab team in a full-time SNF setting. You will evaluate residents within 24 hours of referral, develop and adjust treatment plans, and supervise COTAs and aides to support patient independence and quality of life.

The role requires CA OT licensure, with a master’s or doctorate preferred. Proficiency with Casamba and PointClickCare is valued as part of our documentation workflow.

Qualifications

• Bachelor’s degree in Occupational Therapy is required.
• Current California OT license in good standing; CE requirements.
• Experience in skilled nursing facility or geriatric settings preferred.

Responsibilities

• Evaluate residents within 24 hours of physician referral and document delays.
• Develop individualized treatment plans and obtain physician approval.
• Supervise COTAs, aides and students in compliance with state law.
• Educate residents and families; participate in discharge planning and care coordination.
• Complete evaluations, daily and weekly notes, recertifications, and discharge documentation.
