Job summary

Garden City Health Care Center in Modesto, CA is seeking a Certified Occupational Therapy Assistant (COTA) for a full-time, on-site role in a skilled nursing facility. You will support the OT with treatment plans, document evaluations, and participate in discharge planning to enhance residents’ independence and quality of life.

The ideal candidate is a graduate of an accredited OTA program with CA COTA licensure; geriatric experience is preferred and ongoing CE credits are required.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Licensed as a California COTA in good standing.
• Geriatric or skilled nursing experience preferred.
• Maintain required continuing education credits.

Responsibilities

• Assist the supervising OT with patient care and treatment implementation.
• Follow physician orders and treatment plans; document evaluations and progress notes.
• Record daily treatment notes, weekly progress notes, and recertification documents as required.
• Communicate resident progress and plans with the OT and interdisciplinary team.
• Assist in training Restorative Aides and participate in in-service meetings.
• Educate residents and families on maintenance and discharge planning.
• Contribute to discharge planning and transition of care processes.
• Maintain a clean, safe treatment area and report equipment issues.
