Job summary

Garden City Health Care Center in Modesto, CA is seeking a compassionate Occupational Therapist to join our rehabilitation team. You will evaluate and treat residents, develop and adjust plans of care, and supervise COTAs and aides within a skilled nursing facility setting.

You will educate residents and families, participate in interdisciplinary teams, and assist with discharge planning and coordination. California OT license and solid documentation skills are required; a Master’s or Doctorate

Qualifications

• Bachelor’s degree in Occupational Therapy required.
• Master’s or Doctorate degree in Occupational Therapy preferred.
• Skilled nursing or geriatric experience preferred.

Responsibilities

• Evaluate residents within 24 hours (or one business day) of physician referral.
• Develop individualized treatment plans and obtain physician approval.
• Treat residents according to physician-approved plans of care.
• Supervise COTAs, aides, and students.
• Educate residents, families, and caregivers on maintenance and discharge planning.
• Participate in discharge planning and care coordination.
