Job summary

Renewal Rehab, part of Enhance Therapies, is hiring an Occupational Therapist for a full-time position in Prophetstown, IL. The role offers a competitive salary range of $75,000–$90,000 per year and requires an active OT license in the state.

You will evaluate and treat patients in a skilled nursing/long-term care setting, develop care plans, and maintain compliant documentation while collaborating with a multidisciplinary team.

Qualifications

• Must have OT degree from an AOTA-accredited program.
• NBCOT certification as an OTR if required by the state.
• Current licensure as an Occupational Therapist in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure physician orders are obtained and up to date.
