Job summary

Renewal Rehab is seeking a licensed Occupational Therapist for a full-time role in Prophetstown, IL. This position offers competitive pay and a professional development-focused environment within a skilled nursing facility/long-term care setting.

You will evaluate and treat patients with functional, cognitive, and ADL impairments, develop care plans, and collaborate with team members to ensure quality outcomes while maintaining regulatory documentation and timely physician orders.

Qualifications

• Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program.
• NBCOT certification as an OTR, if required by the state.
• Current license as an Occupational Therapist in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
