Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to serve an in-house Skilled Nursing Facility in Wisconsin. The role involves planning, organizing, developing, and directing Occupational Therapy Services per applicable federal, state, and local standards to ensure the highest quality resident care at all times.

The position offers Full Time or PRN hours and requires an Associate's degree in Occupational Therapy with active OTA licensure, plus ongoing professional

Qualifications

• Associate's degree in Occupational Therapy is required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with required documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Ongoing continuing education and professional development
