Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Greendale, Wisconsin. You will participate in planning, organizing, and directing occupational therapy services to ensure high-quality resident care while following CMS guidelines.

An Associate’s degree and active OTA licensure are required, with ongoing education encouraged. Position is offered as Full Time, Part Time, or PRN with benefits varying by status.

Qualifications

• Associate's degree in Occupational Therapy (required).
• Active OTA licensure in state(s) of practice (required).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention.
• Follow treatment plans and short/long-term goals.
• Communicate evaluation findings to residents and families.
• Establish resident therapy schedules with the team.
• Document and bill occupational therapy services.
• Perform duties to effectively treat patients.
• Comply with CMS documentation standards daily.
