Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Rice Lake, Wisconsin. This PRN role centers on delivering patient-centered OT services under supervision within a facility setting.

You will screen residents, implement treatment plans, document progress, and collaborate with an interdisciplinary team to ensure quality resident care and CMS compliance.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in Wisconsin required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and care teams.
• Establish individual resident therapy schedules with the team.
• Document and bill occupational therapy services per plan of care.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards of practice daily.

Education

Associate's degree in Occupational Therapy
