Job summary

Evolve Therapy Services is a therapy management organization hiring compassionate Certified Occupational Therapy Assistants for an in-house Skilled Nursing Facility located in the region.

The role includes planning, directing Occupational Therapy services, coordinating with the interdisciplinary team, and ensuring quality resident care in compliance with CMS standards. Active licensure and ongoing education are required to maintain practice.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans according to established goals and plan of care.
• Communicate evaluation findings and treatment plans to residents and interdisciplinary team.
• Establish resident therapy schedules with team members.
• Conduct therapy per plan and ensure documentation and billing compliance.
• Perform other duties as required to effectively treat patients.
• Ensure CMS documentation standards are met daily.
