Job summary

Evolve Therapy Services is seeking a compassionate Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Oil City, PA. The role supports the plan, organization, and direction of occupational therapy services in accordance with standards and administrator direction.

Requirements include an Associate's degree in Occupational Therapy and active OTA licensure in the state(s) of practice, plus ongoing continuing education.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Develop and follow treatment plans with supervision of a licensed therapist.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and the care team.
• Create individual resident therapy schedules with team coordination.
• Conduct therapy and document billing in accordance with the treatment plan.
• Perform other duties as assigned to effectively treat patients.
• Adhere to CMS documentation standards daily.
