Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Oil City, PA. This full‑time role (PRN option also available) focuses on delivering occupational therapy services per plan of care and ensuring complete documentation and compliance with CMS standards.

Requires an Associate's degree in Occupational Therapy and active OTA licensure in the state(s) of practice, with ongoing continuing education to maintain licensure.

Qualifications

• Must have OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Execute treatment plans, follow goals set by supervising therapist.
• Communicate evaluation findings and recommendations to residents, families, and the team.
• Schedule resident therapy in coordination with team members.
• Deliver therapy per plan and ensure proper documentation and billing of services.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards and daily point-of-care documentation.
