Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Oil City, PA. The role is full time with a $5,000 sign-on bonus.

Responsibilities include screening residents, implementing treatment plans, coordinating schedules, and ensuring proper documentation and billing per CMS standards. Active OTA licensure in applicable states is required, with ongoing CEU needs.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in the practicing state(s) is mandatory.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Follow treatment plans and goals established with supervising therapist.
• Communicate evaluation findings and plans to residents, families, and team.
• Create individual therapy schedules with team collaboration.
• Deliver therapy, ensure documentation and billing per plan.
• Perform other duties to effectively treat patients as needed.
• Comply with CMS documentation standards daily.
