Responsibilities

• Demonstrates safe patient transfers with assistance as needed.
• Designs and adjusts care plans with goals, treatments, outcomes, and discharge plan.
• Educates patients, families, and caregivers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes of care.
• Performs examinations with reliable methods based on history and screening.
• Documents in EMR per policy and communicates progress with providers.
