Job summary

Carle Health in the United States is seeking an RN Navigator to coordinate surgical patient care across specialties. You will advocate for patients, guide them through diagnosis, treatment and follow-up, and work with interdisciplinary teams to ensure quality outcomes.

The role requires BLS, an active RN license and a BSN, with experience in Orthopedics preferred. You will support program development, quality improvement and patient education initiatives.

Qualifications

• BLS certification within 30 days (AHA).
• RN license in good standing (state).
• BSN required.
• 2+ years in Orthopedics as an RN.
• Demonstrated communication, organization and critical thinking.
• Proficiency in conflict resolution, delegation and time management.
• Proficient with electronic medical records (EMR) and data management systems.
• Ability to work autonomously and collaborate within a team.

Responsibilities

• Serve as liaison among physicians, nursing staff, ancillary staff and patients/families to improve continuity of care.
• Navigate patients through diagnosis, surgical treatment and follow-up to ensure positive outcomes.
• Facilitate referrals, scheduling and follow-up appointments for longitudinal care.
• Apply the nursing process to provide direct patient care as needed.
• Develop and document individualized, patient-centered care plans.
• Deliver holistic care including education and psychosocial support.
• Educate patients and families about diagnoses, treatments, options and resources.
• Create educational materials tailored to patient needs.
• Promote patient empowerment through evidence-based practices.
• Collaborate with leadership and providers to develop clinical guidelines and care protocols.
• Monitor program performance metrics and patient outcomes.
• Participate in community outreach, committees and multidisciplinary clinics.
• Maintain clear communication with patients and the healthcare team.
• Mentor staff, supporting education and competency development.
• Adhere to policies, procedures and regulatory standards.
