Job summary

Carle Health is hiring an RN Navigator to coordinate surgical patient care across specialties. You will advocate for patients, support program development, quality improvement and education initiatives.

Requirements include state RN license, BSN, BLS within 30 days, and at least 2 years in Orthopedics. You will work with EMR and data systems to enhance outcomes and patient experience.

Qualifications

• BLS certification within 30 days of hire.
• RN licensed and in good standing with state board.
• BSN required; 2+ years orthopedic RN experience preferred.
• Strong communication, organizational and critical-thinking skills.
• Proficient with EMR and data management systems.

Responsibilities

• Serve as a liaison between physicians, nursing staff, ancillary staff, and patients/families to enhance continuity of care.
• Navigate patients through diagnosis, surgical treatment and follow-up for a positive outcome.
• Facilitate referrals, scheduling, and follow-up appointments.
• Independently apply the nursing process to provide direct patient care as needed.
• Develop, update and document individualized, patient-centered care plans.
• Deliver holistic care including education and psychosocial support.
• Educate patients and families about diagnoses, treatments, options, medications.
• Create and maintain educational materials tailored to patient needs.
• Promote patient empowerment through evidence-based practices.
• Collaborate with leadership to develop clinical guidelines and care protocols.
• Monitor program performance metrics, patient outcomes and growth opportunities.
• Contribute to community outreach programs and multidisciplinary clinics.
• Maintain clear communication with patients and the healthcare team.
• Serve as a resource and mentor to staff for education and competency.
• Adhere to policies, procedures and regulatory standards.
