Job summary

Carle Health is seeking an experienced RN Navigator to coordinate surgical patient care across orthopedics and other service lines. You will collaborate with physicians, nursing staff, and patients to ensure a seamless care experience from diagnosis through follow-up.

Applicants must be a licensed RN with an active BLS, a BSN, and at least two years of Orthopedics experience. Strong communication and EMR skills are essential to succeed in this role.

Qualifications

• BLS certification within 30 days.
• Illinois RN license in good standing.
• Minimum 2 years RN experience in Orthopedics.
• Strong communication, organization and critical-thinking skills.
• Proficiency with electronic medical records and data management systems.

Responsibilities

• Liaise between physicians, nursing staff, ancillary staff and patients/families to enhance continuity of care.
• Navigate patients through diagnosis, surgical treatment and follow-up for positive outcomes.
• Coordinate referrals, scheduling and follow-ups to optimize care continuum.
• Apply the nursing process to provide direct patient care as needed.
• Develop and document individualized, patient-centered care plans.
• Deliver holistic care including education and psychosocial support.
• Educate patients and families about diagnoses, treatment options and resources.
• Develop educational materials tailored to patient needs.
• Promote patient engagement through evidence-based practices.
• Collaborate on clinical guidelines, policies and care protocols.
• Monitor program performance, quality metrics, and patient outcomes.
• Participate in community outreach and multidisciplinary clinics.
• Maintain clear communication with patients and the healthcare team.
• Mentor staff and support competency development.
• Adhere to policies and regulatory standards.
