Responsibilities

• Deliver direct and indirect patient care per individualized care plans in outpatient setting.
• Administer medications and perform procedures under RN supervision, per NYS rules.
• Observe, record, and report patient health data accurately.
• Communicate care plans, treatment options, and follow-up with patients and families.
• Provide emotional support and health education to patients and families.
• Collaborate with the healthcare team to ensure seamless patient care and effective communication.
• Participate in quality improvement initiatives to enhance patient experiences and outcomes.
