Fairview Health Services in Minneapolis, MN, is hiring an RN Care Coordinator for the Orthopedic Specialty Clinic on the University of Minnesota campus. This full-time, salaried role focuses on coordinating care for orthopedic patients across the health continuum and providing education to patients, families, and staff.

Qualified candidates will have RN licensure in Minnesota, 3–5 years of specialty care coordination or equivalent nursing experience, and BLS certification.

Qualifications

• 3 years of specialty care coordination experience or 5 years RN clinical experience
• MN RN license required
• BLS certification required
• On-site role at Fairview Clinic and Surgery Center in Minneapolis

Responsibilities

• Accesses, analyzes, plans and provides follow-up care for orthopedic surgery patients
• Facilitates timely referrals and builds positive relations with community providers
• Evaluates progress via calls, test results and report preparation
• Acts as liaison between patient/family and medical staff to promote patient-centered care
