Job summary

The Jewish Community Center of Staten Island (JCC) in New York seeks a dedicated Case Manager to support individuals through OPWDD programs and collaborative services. You will handle intakes, enrollments for afterschool and weekend respite services, day habilitation, camp, employment services, and community engagement, ensuring individualized planning and quality care.

You will coordinate with the Senior Director to ensure enrollment, reporting, and compliance documentation is completed timely

Qualifications

  • Valid NY State Driver's License is required.
  • Experience in a human service-related environment.
  • 2 years' experience working with the Special Needs Population.
  • Knowledge of OPWDD Policies.

Responsibilities

  • Oversees client care by assessing treatment needs during intake and enrollment; develops, monitors, and evaluates individualized treatment plans; facilitates interdisciplinary collaboration; and monitors staff performance.
  • Manages the admission of new clients by reviewing records and applications and conducting comprehensive orientations.
  • Determines client needs through intake interviews; coordinates evaluations and reviews treatment objectives and care plans.
  • Establishes and coordinates treatment programs by developing schedules and routines; oversees service delivery and arranges resources.
  • Monitors ongoing cases by tracking attendance; evaluates treatment effectiveness; advocates for services; and provides ongoing client support.
  • Maintains accurate and compliant client records by reviewing case documentation and recording progress updates.
  • Communicates client progress through weekly interdisciplinary meetings and formal evaluations; shares outcomes and barriers with clinical teams and families.
  • Facilitates client transition by strengthening transition plans; coordinating post-transition services; training family members; and providing community resources.
  • Improves treatment outcomes by analyzing, evaluating, and redesigning programmatic processes; updating policies and procedures.
  • Prepares comprehensive reports by collecting and summarizing treatment outcomes and trends; compiling data and grant/subsidy applications.
  • Maintains professional expertise by continuing education and networking with peers.

Education

BA/BS degree in Special Education, Social Work, Psychology- or any other Human Service-related field
