Qualifications

  • OD degree from an accredited School of Optometry.
  • Optometrist license (California) at hire.
  • Therapeutic Pharmaceutical Agents (TPA) required for external employees hired after November 2009 (TPA not required for internal applicants).
  • Must have Natl Provider Identifier (NPI) or obtain one prior to start date.

Responsibilities

  • Take patient history including chief complaints, symptoms, ocular and medical history.
  • Perform and document testing using ophthalmic equipment; document findings.
  • Interpret data and develop impressions and plan; document and refer as needed.
  • Collaborate with optical and ophthalmology teams; participate in staff meetings and ensure quality care.
  • Maintain licensing and mandatory training; complete required forms and communications.
