Job summary

Eyetastic Services in Rockford, IL is seeking a talented optometrist to join a growing practice. You will provide full-scope optometry, conducting eye exams, contact lens fittings, LASIK consultations, and disease screening in a collaborative, patient-focused environment.

This role offers a base salary of $150,000–$170,000 with additional monthly incentives and phantom equity opportunities, plus medical/dental coverage and PTO.

Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license or in the process of obtaining.

Compensation and Benefits

• Total Potential Annual Earnings: ~$200,000 (including phantom equity)
• Base Annual Salary: $150,000 - $170,000
• Monthly Incentive Bonus: Averaging $1,000 - $1,500
• Generous Retention Bonus: $10,000 - $20,000 based on the length of your employment agreement
