Northwell in Connecticut is seeking a registered nurse to provide age-appropriate care using the nursing process across the patient care unit. You will collaborate with physicians and health-team members to coordinate procedures and treatments, lead care planning, and supervise care activities.

The role emphasizes leadership, clinical judgment, accurate documentation, and ongoing patient and family education. A valid RN license in NYS or CT and BLS certification are required.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut.
• BLS certification required; other certifications as needed.

Responsabilités

• Assess the patient’s physiologic health status and review charts.
• Assess gastrointestinal, cardiovascular, respiratory, renal and neurological health.
• Identify patient needs for teaching and set individualized goals.
• Formulate and prioritize the plan of care to achieve outcomes.
• Implement care, monitor safety, and document care and patient responses.
• Reassess and revise plans as needed; administer treatments.
• Collaborate with physicians and health team members for care plans.
• Perform grade I-IV Decubitus care and skin care.
• Administer medications safely and accurately; educate patients.
• Participate in discharge planning and referrals as appropriate.
• Perform related duties as required.
