Qualifications

Responsibilities

• Assess the patient’s condition and nursing needs; coordinate nursing care with other health disciplines to align with treatment plan.
• Develop individualized care plans including teaching needs and discharge planning.
• Administer direct patient care and medications.
• Practice infection control and maintain a clean, safe environment.
• Communicate changes in patient condition to Charge RN/Clinician.
• Document patient education and responses in medical records.
• Collaborate with staff across departments; participate in unit/hospital quality initiatives.
