Responsibilities

• Collaborates with patients and families to identify learning needs and provide education.
• Uses health IT to communicate and monitor care outcomes and drive safety improvements.
• Works within nursing and inter-professional teams with open communication and shared decision making.
• Supports preceptor role and opportunities for staff development.
• Participates in safety and quality improvement committees.
• Delivers patient-centered care respecting preferences and cultural beliefs.
