Job summary

UChicago Medicine is seeking a Special Procedures Nurse (RN) to join the Care Transition Clinic at our Hyde Park onsite location. The role requires coordination of care across departments for patients and potential patients in the community, under faculty supervision.

The RN will provide technically complex nursing care, including sedation and high-risk procedures, and may have charge nurse responsibilities. This is a 0.6 FTE, part-time, onsite position in the greater Chicagoland area.

Qualifications

• RN required.
• BSN preferred; 5+ years clinical experience may be considered in lieu of degree.

Responsibilities

• Coordinates population health care needs across the care continuum.
• Acts as a clinical liaison for patients and potential patients across departments and locations.
• Performs nursing procedures and high-risk care, including sedation and monitoring during procedures.
• Maintains patient assessments, documentation and communication with physicians to support timely interventions.
• Rotates to clinical areas to assist with staffing and safe patient care.
