Job summary

Visante LLC seeks an experienced Pharmacist to join our onsite health system team. You will verify pharmacotherapy, review medication regimens, monitor therapy, and provide patient education to ensure safe and effective medication use. The role blends direct patient care with interdisciplinary collaboration and quality improvement.

Requirements include Pharm.D. (ACPE), Indiana pharmacist license, 3 years of retail or specialty pharmacy, and BLS; ASHP PGY1 or residency preferred.

Qualifications

• Bachelor's in Pharmacy or Pharm.D. from an ACPE-accredited school of pharmacy
• 3 years of retail or specialty pharmacy experience
• Current Pharmacist license
• ASHP-accredited PGY1 or specialty residency preferred

Responsibilities

• Oversee medication therapy management in alignment with policies.
• Identify, report, and resolve medication errors and safety issues.
• Complete timely and accurate documentation per policies.
• Collaborate with interdisciplinary teams to enhance patient care and program development.
• Support quality improvement initiatives and cost-savings efforts.
• Review and optimize medication regimens to improve outcomes
