Visante LLC is seeking a Pharmacist to join our onsite health system pharmacy team in Richmond, VA. The role emphasizes pharmacotherapy verification, patient education, and collaboration with interdisciplinary teams to improve outcomes, safety, and cost-effectiveness.

The candidate will review and optimize medication regimens, participate in quality initiatives, and ensure compliance with regulatory standards while maintaining patient-centered care.

Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy.
• 3 years of retail or specialty pharmacy experience.
• Current Pharmacist license as granted by the appropriate state licensing authority.
• BLS certification from American Heart Association obtained within 6 months of hire date.
• Board of Pharmacy Specialties certification (preferred).

Responsibilities

• Verify pharmacotherapy to enhance treatment efficiency, effectiveness, and cost-effectiveness.
• Review and optimize medication regimens and address drug-related issues.
• Provide patient education to ensure safe and effective medication use.
• Collaborate with interdisciplinary teams and participate in quality improvement initiatives.
• Identify and resolve medication errors and safety issues.
• Ensure compliance with clinical standards and Visante procedures.
