Visante Consulting LLC seeks an Onsite Pharmacist in Colorado to join a health-system pharmacy team. You will support specialty and ambulatory oncology settings, collaborate with oncology providers, and deliver patient education for continuous, safe care.

The role emphasizes clinical practice, education, and quality leadership, with a focus on medication safety and cost-effectiveness. Requires PharmD/BS Pharmacy and Colorado licensure, with 3 years of retail or specialty pharmacy experience.

Competenze

• Requires Bachelor’s or PharmD from ACPE-accredited school.
• ASHP-accredited PGY1 or specialty residency preferred.
• 3 years of retail or specialty pharmacy experience required.
• Current Pharmacist license in the appropriate state, with BLS within 6 months of hire.
• Board certifications (BCOP/BCACP/BCPS/CSP, etc.) preferred.

Mansioni

• Oversee medication therapy management per policies.
• Identify, report, and resolve medication errors and safety issues.
• Respond to changing department needs and tasks from leadership.
• Complete timely, accurate documentation per policies.
• Contribute to Quality Improvement initiatives and maintain records.
• Provide patient-centered medication therapy with confidentiality.
• Collaborate with interdisciplinary teams to improve patient care.
• Support initiatives to improve safety, efficiency, and cost-effectiveness.
• Optimize medication use by resolving drug-related problems.
