Job summary

Qualfon in Casselberry, FL is hiring a Telehealth Care Coordinator (Onsite) to coordinate nutrition therapy for pediatric patients with complex medical needs and to support care plans involving enteral feeding.

You will work closely with clinicians, dietitians and DME suppliers to ensure continuity of care, monitor adherence and guide families through transitions of care and growth-related changes, all on a consistent weekday schedule.

Qualifications

• High school diploma or GED required; college coursework a plus.
• 1+ year of customer service experience
• Typing speed of 25 WPM with 90% accuracy
• Proficiency in MS Office: Word, Outlook & Excel
• Strong written and verbal communication skills

Responsibilities

• Coordinate nutrition therapy for pediatric patients with complex needs.
• Support care plans involving enteral feeding and medical nutrition.
• Partner with clinicians, dietitians, and DME suppliers.
• Monitor adherence, supply continuity, and clinical changes.
