Job summary

Brooksby Village by Erickson Senior Living in Peabody, MA is seeking a Per Diem on-site Out-patient Physical Therapist to support our residents' mobility and quality of life. You will assess, treat, and implement individualized care plans within an interdisciplinary team.

Responsibilities include applying therapeutic treatments for older adults across multiple diagnostics, completing required documentation for reimbursement and regulatory purposes, and ensuring compliance with state licensure

Qualifications

• Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide outpatient physical therapy to residents with a holistic approach.
• Implement residents' personalized care plans focusing on strengths and preferences.
• Collaborate with the interdisciplinary team to address care needs and ensure regulatory compliance.
