Job summary

Seabrook Village by Erickson Senior Living is hiring a per diem Physical Therapist to support our Home Care Department, serving independently living residents within our campus. The role emphasizes resident-centered care, holistic support, and collaboration with an interdisciplinary team.

A valid PT license and experience with older adults are preferred; OASIS experience is a plus. Onsite benefits and per diem compensation apply.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• The degree and training must meet current Medicare and state requirements.

Responsibilities

• Provide physical therapy to residents within the Seabrook Village campus.
• Implement residents' personal care plans with a focus on function, safety, and comfort.
• Complete documentation to meet reimbursement and regulatory requirements.
• Collaborate with the interdisciplinary clinical team to address residents' needs.
