Job summary

CareCore Health LLC is seeking an experienced MDS Coordinator for an onsite role in Ohio. You will lead MDS assessments, ensure compliant care plans, and collaborate with nursing, therapy, dietary, and social work teams to improve resident outcomes.

The ideal candidate has an active RN or LPN license (RN preferred) and prior MDS experience in long-term care. PCC knowledge and RAC-CT are assets. This role emphasizes accuracy, teamwork, and ongoing quality improvement.

Qualifications

• Active RN or LPN license required; RN preferred.
• MDS Coordinator experience in long-term care or skilled nursing setting required.
• RAC-CT certification preferred.
• Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.
• Strong assessment, organizational, and critical thinking skills.
• Experience with PointClickCare (PCC) preferred.
• Excellent communication and teamwork abilities.

Responsibilities

• Complete and coordinate MDS assessments in accordance with state and federal regulations.
• Ensure accurate and timely completion of care plans and required documentation.
• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
• Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.
• Participate in quality improvement initiatives and survey preparedness.
• Educate and support staff regarding MDS processes and documentation requirements.
