Job summary

CareCore Health LLC in Lima, OH is seeking a dedicated and experienced full-time MDS Coordinator to join our team. On-site position with competitive wages and a supportive work environment.

The MDS Coordinator will ensure compliance with federal and state guidelines for MDS, manage Medicaid and Medicare RUGs/PDPM, and oversee documentation. Strong knowledge of Quality Measures and Five-Star Rating is essential. RN license preferred.

Qualifications

• Licensed nurse in Ohio and valid licensure.
• Minimum 1 year of MDS Coordinator or equivalent experience.
• Strong knowledge of MDS 3.0 guidelines and SNF regulations.
• Understanding of Medicare/Medicaid reimbursement processes.
• Excellent communication, organization, and time management.
• Proficient with EHR and MDS software; team player.

Responsibilities

• Ensure compliance with federal/state MDS guidelines.
• Manage Medicaid/Medicare RUGs/PDPM and Case Mix.
• Meet Medicare criteria with required documentation.
• Understand Quality Measures and Five-Star ratings.
• Collaborate with care teams to optimize resident care.
