Job summary

Seen Health, Inc. is seeking a Social Work Assistant in Los Angeles to support the interdisciplinary team in delivering compassionate elder care. You will assist with care plans, coordinate community resources, and maintain documentation for program participants.

The role requires engaging with families, providing psychosocial support, and ensuring timely tasks and data entry. On-site work with competitive hourly compensation and comprehensive benefits is offered.

Qualifications

• High School Diploma or Equivalency.
• Associate's Degree preferred in Social Work/Human Services/Social Science.
• Valid CA Driver's License with clean DMV record.
• Minimum 1 year experience with frail/elderly population (3+ years preferred).
• CPR certification required or ability to obtain.

Responsibilities

• Support Social Workers with daily tasks and escalate as needed.
• Establish rapport with participants and families for psychosocial support.
• Assist with IDT care plan reviews within regulatory timeframes.
• Document and deliver social work interventions per care plans.
• Transcribe meeting notes and provide admin support to MSW and IDT.
