Responsibilities

• Admit new clients by reviewing records and conducting orientations.
• Schedule and coordinate services, including transportation arrangements.
• Monitor cases, advocate for services, and obtain additional resources.
• Maintain client records by reviewing notes and progress.
• Communicate progress via weekly meetings with the therapeutic team.
• Prepare discharge plans and coordinate post-discharge resources.
• Compile reports and grant/subsidy applications with data.
