Position Summary Dhaka Technologies Limited Company (DTLC) is hiring a Case Manager on behalf of a government client in Washington, DC. This is a full time, onsite position working 8 hours per day. Duties and Responsibilities The Case Manager admits new clients by reviewing records and applications; conducting orientations; completing intake interviews; and determining the need for referrals and interventions. The Case Manager assists clients by setting schedules and routines; coordinating services being provided; arranging resources, including the coordination of transportation for clients. The Case Manager monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing advice and personal support to clients and families. The Case Manager also shall maintain clients' records by reviewing case notes; logging events and progress. Additionally, the Case Manager communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to a therapeutic team and family members. The Case Manager prepares for a client's discharge by reviewing discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; and providing resources. The Case Manager prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; and completing grant and subsidy applications. The Case Manager updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations. The Case Manager shall be able to perform all required duties at any service location operated under this contract.
Minimum Requirements High school diploma or GED Knowledge of case management practices, client intake and assessment, service coordination, crisis intervention, and client documentation 1 to 3 years of relevant experience Residency Preference District of Columbia residents will receive special preference points in the selection process.
