Job summary

Dhaka Technologies Limited Company (DTLC) is seeking a Case Manager (VWRC) for a Washington, DC government client. It is a contract, full-time, onsite role with 40 hours per week, Monday–Friday, 8 hours daily.

The Case Manager will admit new clients, coordinate services including transportation, monitor progress, maintain records, conduct weekly team meetings, plan discharges, and generate data reports. Local DC area residency is required.

Qualifications

• Knowledge of case management practices, client intake and assessment, service coordination, crisis intervention, and client documentation.

Responsibilities

• Admit new clients by reviewing records and applications; conduct orientations; complete intake interviews; determine referrals and interventions.
• Assist clients by setting schedules and routines; coordinating services; arranging resources including transportation.
• Monitor cases by verifying attendance; observe and evaluate treatments; advocate for services; obtain resources; intervene in crises.
• Maintain clients' records by reviewing case notes; logging events and progress.
• Communicate clients' progress by weekly interdisciplinary meetings and disseminating results to the team and family.
• Prepare for discharge by reviewing discharge plans; coordinate discharge and post-discharge requirements; orient and train family; provide resources.
• Prepare reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant/subsidy applications.
• Perform all required duties at any service location under this contract.
