Job summary

Avail Health seeks a compassionate Care Navigator/Enrollment Partner to serve as the onsite representative within a Rockville hospital. You’ll identify eligible patients, explain the Care Transitions Program, facilitate enrollment, and ensure discharge with a clear plan and follow-up.

You’ll build trust with patients and hospital teams, coordinate with interdisciplinary staff, and drive enrollment while maintaining accurate documentation and excellent patient experience in a fast-paced

Qualifications

• Two+ years of experience in healthcare patient engagement, care coordination, patient access, or related operations.
• Experience enrolling patients into healthcare programs or value-based care initiatives.
• Excellent interpersonal, organizational, and time-management skills.
• Comfortable using EMR systems, scheduling platforms, and Microsoft Office.

Responsibilities

• Serve as Avail Health’s primary onsite representative within the assigned hospital.
• Identify eligible patients and enroll them into the Care Transitions Program before discharge.
• Explain the program and address questions to build patient confidence.
• Coordinate enrollment documentation and obtain required consents.
• Schedule initial Care Transitions appointment prior to discharge.
• Maintain accurate documentation in hospital systems and Avail Health’s platforms.
• Collaborate with interdisciplinary teams to improve workflows and patient engagement.
