Job summary

Avail Health is seeking a compassionate Care Navigator to serve as the primary onsite representative within a Rockville-area hospital. You will identify eligible patients, enroll them in the Care Transitions Program, and ensure a clear discharge plan with a scheduled follow-up appointment.

You will build trust with patients and hospital teams, coordinate enrollment, and help shape workflows to improve engagement and outcomes for older adults with complex needs.

Qualifications

• Experience in healthcare patient engagement or care coordination.
• Experience enrolling patients into healthcare programs or value-based care initiatives.
• Strong interpersonal skills and compassionate patient-centered care.

Responsibilities

• Serve as Avail Health’s primary onsite representative within the hospital.
• Identify patients who may benefit from the Care Transitions Program.
• Explain the program, enroll patients, and obtain required consents.
• Schedule initial Care Transitions appointment before discharge.
• Coordinate handoffs between hospital staff and the care team.
• Maintain accurate documentation in hospital systems and EHR.
• Contribute to workflow improvements and program growth.
