Qualifications

• Minimum 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Current, unrestricted RN license in the state of South Carolina.
• Valid driver’s license and proof of personal vehicle liability insurance.

Responsibilities

• Perform comprehensive, face-to-face assessments of newly admitted members at acute care facilities.
• Educate members and families on discharge planning from admission to post-discharge.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement solutions.
• Collaborate with providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures to promote wellness.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety, and privacy standards.

About This Role

• **Schedule:** Full-time, Monday through Friday, 8:00 AM – 5:00 PM local time

What You'll Do

• Conduct comprehensive, face-to-face assessments of newly admitted members at assigned acute care facilities.

Requirements

Education:

• Current, unrestricted Registered Nurse (RN) license in the state of South Carolina

Experience:

• Minimum of 3 years recent acute care nursing experience
• Experience with discharge planning or care coordination in a hospital or health plan settings

Licenses & Certifications:

• Current RN License in South Carolina required
• Valid driver’s license and proof of personal vehicle liability insurance
