Humana is hiring an Onsite Care Coordination Nurse in Illinois to support members during critical moments in their care journey. You will conduct face-to-face assessments at acute care facilities, lead discharge planning, and collaborate with providers and care teams to ensure seamless transitions.

The role requires an active RN license in Illinois and at least 3 years of acute care experience, with strong skills in care coordination, patient education, and documentation.

Qualifications

• Current, unrestricted RN license in Illinois.
• Minimum of 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Valid driver's license, dependable automobile, and proof of personal vehicle liability insurance.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement strategies to address these issues.
• Collaborate with attending providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures to promote ongoing wellness.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control, safety, and patient privacy standards.

About This Role

As an Onsite Care Coordination Nurse at Humana, you will support members during critical moments in their care journey by providing face-to-face assessments and leading care coordination efforts in acute care settings. • Schedule: Full-time, Monday through Friday, 8:00 AM - 5:00 PM
• Pay: $71,100 - $97,800 per year
• Sign-On Bonus: Eligible for a bonus incentive plan

What You'll Do
