Job summary

The Floating Hospital in New York City seeks a capable administrative liaison to connect DHS shelter services with the Families in Temporary Housing (FTH) team. You will manage referrals, schedules, and communications to ensure smooth care pathways for patients.

Responsibilities include coordinating with DOE staff, processing new medical referrals, and maintaining accurate patient data. Strong Excel skills and public speaking abilities are essential, with bilingual English/Spanish preferred.

Qualifications

Responsibilities

• Serve as a liaison between DHS shelter personnel and Families in Temporary Housing (FTH) team.
• Serve as a liaison between DOE personnel (social workers, guidance counselors, school nurses, teachers) and the FTH team.
• Process referrals on TFH online platform and create accounts and appointments for new medical patients.
• Assist Registration to ensure daily medical provider schedules have correct New Visits.
• Review dental referrals and coordinate scheduling with the Dental team.
• Review mental health referrals and place ATHENA alerts when scheduling medical appointments.
• Ensure patient demographics and insurance data from referrals are current.
• Maintain FTH referral database online on TFH server.
• Assist the Director of FTH with other projects as needed.
• Monitor daily DHS New appointments to meet quota.
• Promote workflow by clarifying duties and delegating tasks.
• Distribute online referrals to staff as needed.
• Create/update communications for FTH protocol changes.

Job Description

• Serves as a liaison between DHS shelter personnel and Families in Temporary Housing (FTH) team
• Serves as a liaison between DOE personnel (e.g., social workers, guidance counselors, school nurses, teachers) and the FTH team
• Processes all referrals made on TFHs on-line referral platform and creates accounts and appointments for new medical patients
• Works with the Registration department to ensure that the daily medical provider schedules are populated with the correct number of New Visits
• Reviews new referral requests for dental care and coordinates/ sends information to Dental team for scheduling
• Reviews new referral requests for mental health (that are made in conjunction with requests for medical appointments), and places alert in ATHENA for medical provider at time that the medical appointment is being scheduled
• Ensures that all patient demographic and insurance information from shelter and school referrals is current and accurate
• Maintains FTH referral database online on TFHs server
• Assists the Director of FTH with other projects as needed
• Daily monitoring of "DHS New" scheduled Appointments to ensure quota is met
• Promote a healthy workflow by ensuring department understands duties and by delegating tasks as needed
• Distribution of Online referrals to dept staff as needed
• Create and/or update correspondences to ensure FTH team is made aware of any changes to protocols that require implementation.

Requirements

• This position requires expertise in computer systems including Microsoft Office applications, Athena Medical Records System, and Online portal and referral systems.
• A qualified candidate should be able to download information from web pages, put information on spreadsheets, populate spreadsheets with on-line referrals, and generate reports using Excel.
• The candidate must be comfortable speaking over the phone, in public and addressing large audiences, and have the ability to explain and teach concepts to others through individual follow-up, trainings and presentations.
• Bilingual is preferable (English/Spanish).
• A High School diploma is required and Associate’s or bachelor’s degree preferred.
• The ideal candidate must effectively manage multiple tasks and have excellent writing/grammar skills.
