Job summary

SummitWest Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services in Mesa County homes. This part-time, PRN role offers flexibility, mileage reimbursement, and an opportunity to grow into a full-time position.

You’ll work one-on-one with patients, perform wound care and medication support, maintain accurate documentation, and collaborate with a caring clinical team dedicated to safe, independent home health care.

Qualifications

• Colorado LPN license currently active and in good standing.
• IV certification preferred but not required.
• Strong communication and documentation skills.
• Ability to work independently in home health settings.

Responsibilities

• Provide skilled nursing in patient homes.
• Wound care, medication support, and patient monitoring.
• Observe and report changes or concerns promptly.
• Document visits accurately in the EMR.
• Collaborate with RNs, therapists, caregivers, families, and providers.
• Support patient education and individualized care plan goals.
