Job summary

Allina Health seeks a Home Care Nursing Assistant to provide one-on-one patient care in patients' homes across the West Metro area. You will support independence and safety, build strong patient relationships, and collaborate with a dedicated care team to improve daily living at home.

The role emphasizes compassionate care, documentation accuracy, and adherence to regulatory requirements, with a 8-hour day shift and occasional weekend rotation.

Qualifications

• Must be 18 years of age with education and/or experience to meet required functional competencies as listed in the job description.
• 0 to 2 years as a Nursing Assistant or Home Health Aide.

Responsibilities

• Provide safe, competent care including bathing, skin care, feeding assistance, and bowel program support.
• Assist with ROM, exercises, and household activities.
• Document all patient visits and update supervising clinician on changes in condition.
• Attend staff/home health aide meetings and complete required documentation within 24 hours.
• Comply with agency, state, and federal continuing education requirements.
