Qualifications

• MSW from CSWE-approved school.
• FL LCSW or out-of-state LCSW with plan to obtain FL LCSW within 6 months of hire.
• Minimum 2 years post-MS W experience in social work.
• Preference: 1 year oncology social work experience.
• Strong written and verbal communication skills; competence with computer applications; annual competencies per department checklist.

Responsibilities

• Provide clinical social work services to patients and families.
• Perform social work assessments, counseling, goal setting, advocacy and care plan development.
• Coordinate referrals to outside agencies and continuity of care.
• Collaborate with multidisciplinary team to optimize patient outcomes.
