Qualifications
- Master's Degree: Degree as Social Worker from an accredited School of Social Work.
- Upon hire: Alaska Masters Social Worker License or Alaska Clinical Social Worker License.
- 2 years Medical Social Work experience preferred.
- Experience with computerized documentation and medical records preferred at time of hire.
Responsibilities
- Provide social work services to patients/residents and their families to meet social and emotional needs.
- Collaborate with multidisciplinary team to ensure safe and coordinated transitions of care.
