Responsibilities

• Conducts psychosocial assessments identifying emotional and social needs.
• Provides individual, family, and group counseling (in-person and telehealth).
• Delivers crisis intervention and supportive counseling for oncology issues.
• Develops goal-directed care plans with the multidisciplinary team.
• Utilizes evidence-based therapies and documents all care activities.
• Participates in distress screening and advance care planning.
• Addresses risk management and coordinates with hospital/community agencies.
• Ensures timely, compliant documentation.
