Job summary

Union Health seeks a Social Worker to join the Oncology Services team in Terre Haute, IN. This full-time day shift role focuses on coordinating care, supporting patients and families, and working with a multidisciplinary team to improve outcomes.

You will review records, arrange transportation and financial assistance, and participate in care rounds, discharge planning, and community resources, while guiding patients through their options with empathy and advocacy.

Qualifications

• Strong communication skills with excellent verbal and written abilities.
• Solid knowledge of clinical social work and Indiana State Laws.
• Indiana Social Worker Certification preferred.
• Five years of clinical experience within the last seven years.
• Experience in Case Management strongly preferred.

Responsibilities

• Acts as liaison with the patient and family and the hospital.
• Collaborates with providers, interdisciplinary care team, and community agencies.
• Reviews medical records and interviews patient and/or family; assesses needs.
• Helps coordinate plans of care including physical, financial, and psychosocial needs.
• Supports the Care Manager with complex discharge needs.
• Coordinates care for women's and children's services including guardianship.
• Coordinates with Care Manager to modify interventions as applicable.
• Communicates with the OP Care Team when patients transition out of the hospital.
• Participates in Care Coordination Rounds and Inter-professional Rounds daily.
• Maintains documentation in medical records and department systems.
• Supervises social work students as requested.
• Shows initiative, adaptability, and dependability.
