Job summary

Astera Cancer Care in New Jersey is seeking a Social Worker to join a multidisciplinary hematology/oncology team. The role focuses on psychosocial screening, case management, and survivorship planning for patients and families at various disease stages.

The position includes counseling, resource referrals, and participation in quality programs; travel between NJ locations may be required. A strong background in oncology and bereavement support is essential.

Qualifications

• Licensed Clinical Social Worker (LCSW) required.
• Master’s degree in Social Work (MSW) from a CSWE program required.
• OSW-C certification preferred.
• Spanish-speaking applicants preferred.

Responsibilities

• Conduct psychosocial assessments and develop plans of care.
• Coordinate with physicians, nurses and treatment teams to optimize patient care.
• Provide counseling and education to patients and families.
• Facilitate support groups and bereavement counseling for caregivers.
• Document assessments and treatment plans in the EMR.
• Support and coordinate end-of-life and survivorship care.
